Kindergarten Application
Kindergarten applications are accepted throughout the year. If the class is full, we will place your name on a wait pool.
To apply, most information can be filled out online through the online application form which is available here. The following items will need to be obtained separately and delivered or mailed to the school’s administrative office:
- Pre-School Information Request Form (to be mailed or faxed by the preschool)*
- Copy of baptismal certificate (Catholics only) can be uploaded through the online application form (link is above)
- Copy of custody decree (if applicable) can be uploaded through the online application form (link is above)
- Copy of birth certificate (Original birth certificate must be presented to school personnel for verification before admission is guaranteed) can be uploaded through the online application form (link is above)
- $100 non-refundable application fee (checks payable to Queen of Apostles School)
*Pre-school attendance is not mandatory. If your child has not attended pre-school, please make a note on these forms and we will contact you to schedule a developmental assessment.

Grades 1–8 Application
Applications for grades 1–8 are accepted throughout the year. If a class is full, we will place your name on a waiting list.
To apply, most information can be filled out online through the online application form which is now available here. The following items will need to be obtained separately and delivered or mailed to the school’s administrative office:
- Release of Student Records form (to be mailed or faxed by student’s current school)
- Teacher Recommendation Form (We need two recommendation forms per child.)
- Current year’s report card, including comments, and previous academic year’s report card
- Current standardized test scores plus scores from previous two years, if available
- Copy of baptismal certificate (Catholics only) can be uploaded through the online application form (link is above)
- Copy of custody decree (if applicable) can be uploaded through the online application form (link is above)
- Copy of birth certificate (Original birth certificate must be presented to school personnel for verification before admission is guaranteed) can be uploaded through the online application form (link is above).
